Insurance and Billing
Dr. Kyle Keene is considered an out-of-network provider for all insurance plans and a non-preferred Medicare provider.
At the end of your appointment, you will receive a superbill. The superbill includes all of the information your insurance provider needs to reimburse you. A superbill does not guarantee that an insurance provider will reimburse your healthcare expense.
Prolotherapy and PRP are NOT covered by any insurance including Medicare.
Medicare Part B: Please make sure to bring your Medicare and secondary card to your appointment. You will pay the cost of the appointment after treatment. Dr. Keene will then submit your claim to Medicare. Please expect reimbursement of Medicare claims to take 6-8 weeks and your secondary claim another 4-6 weeks. If your secondary insurance reimburses the practice, we will issue a check. Checks are issued once monthly.
Medicare Part C / Medicare HMO / Medicare PPO / Medicare Advantage Plans: Dr. Keene will provide you with a superbill to submit to your plan
Medi-Cal/ Medicaid: We do not accept or bill Medi-Cal, Medicaid or Medi-Cal related insurance companies. If you have Medi-Cal as your secondary to a Medicare Part B plan please note they typically do not cover the 20%.
Car Accident Claims: We do not bill car insurance companies. You may request a super bill at the end of your appointment and submit your claim directly.

What to wear
It is best to dress comfortably in flexible athletic or athleisure wear. When in doubt think yoga or cozy clothes. Depending on the area of your body being treated, you may choose to wear:
- Treatment of the Neck/Back/Shoulder/Elbow/Hand/Wrist (Sports bra for females)
- Treatment of the Hip/Knee/Ankle/Foot (Loose shorts allowing ability to see and treat above the knee)
Please refrain from wearing jeans, skirts, dresses or other tight clothing. To ensure your treatment is not interrupted please plan to empty all pockets and remove belts, jewelry and shoes prior to treatment.
When to arrive
Please arrive 10-15 minutes prior to your appointment to find a parking spot and get oriented.
All forms must be completed online prior to your appointment start time. We cannot submit Medicare Part B claims if patient registration forms are incomplete.
Patient registration will be sent to you via CERBO, our medical records software. Please ensure forms are completed to the best of your knowledge.
Medicare patients – To ensure your claims can be submitted to Medicare please upload your Medicare and secondary card at the time of registration.
What to expect at your first OMM consult
During your initial visit you will first discuss your medical history followed by a hands-on exam. To help diagnose your concerns, Dr. Keene will ask you to describe and show him what is bothering you. Be prepared to participate in an active physical examination that includes communicating and demonstrating any movement(s) that is/are uncomfortable. The hands on exam may occur while you are sitting, standing or lying down.
Once Dr. Keene has completed his assessment he will discuss your treatment plan and if time allows complete your first treatment.
Following your appointment
Each treatment will affect you differently. You may feel anything from immediate relief to a slight soreness.
Your body needs time to adjust and integrate the changes made during treatment. Participating in either strenuous or sedentary activity can undo the changes made to the body during your treatment.
To maximize the benefits of your treatment after your appointment plan to take it easy but remain reasonably active for the first 24 hours post-treatment. It is best to avoid strenuous activity such as running, contact sports, aerobics or lifting (including groceries, pets and children). It is also best to avoid prolonged sedentary activity such as sitting at a computer, on a long flight or in a dentist chair.
Please schedule physical therapy, fitness training and other body-work appointments until after 24 hours post-treatment unless advised otherwise.
